We at Heart to Heart Disability Support Pty Ltd are looking for Experienced Support Workers to join the team on a casual basis. Work location - Sydney , NSW What you'll be doing : -Record the progress of each client in line with relevant documentation -Provide transport for customers or assistance to use public transport. - Provide physical assistance as required. This may include: -Assistance with meals -Assistance with toileting, bathing etc. -Transfer /hoisting between toilet, wheelchair, motor vehicle, etc. -Medical/special procedures where required -Administering medication where required -Physical assistance to participate in activities -Housekeeping duties -Devise and facilitate program activities and workshops in conjunction with customers and other support staff -Support and actively encourage customer’s participation in decision-making about their own program and about the service Complete documentation as required. This may include: a. Detailed Progress notes b. Shift reports c. Learning Logs / Monthly reports d. Risk management / Positive behavior support Essential Requirement : ABN only Full time / Part time availability Student visa holders are welcome to apply . Job Purpose: To provide client-centred Occupational Therapy services aimed at improving the independence, functionality, and quality of life of individuals across the lifespan. Duties and Responsibilities: - Conduct comprehensive assessments of clients’ physical, cognitive, and psychosocial needs using standardised tools and clinical judgement. - Develop, implement, and regularly review individualised therapy plans that promote clients’ functional independence and participation in daily activities. - Provide intervention strategies including rehabilitation, skill development, environmental modifications, and assistive technology recommendations. - Assess functional limitations of clients resulting from disability, educate and support them in the use of therapeutic strategies and adaptive equipment. - Maintain accurate and timely clinical documentation and progress reports in accordance with organisational and regulatory requirements. - Deliver services in various settings including homes, clinics, schools, aged care facilities, or community environments. - Ensure compliance with the Occupational Therapy Board of Australia and AHPRA standards, as well as relevant legislative frameworks. - Work collaboratively with clients, families, caregivers, health professionals and other support networks to achieve shared goals. - Report progress and status to appropriate parties, which may include family members, carers, teachers, or other health professionals. - Participate in team meetings, professional development programs and contribute to the continuous improvement in quality of services. Skills and Specifications: - Bachelor or Master of Occupational Therapy, or recognised equivalent with minimum 2 years of proven experience. - Current full Australian Health Practitioner Regulation Agency (AHPRA) registration as an Occupational Therapist. - Demonstrated experience in a similar role, with a particular focus on delivering occupational therapy services to children. - Excellent communication and interpersonal skills, with the ability to work collaboratively with a multidisciplinary team. - Must hold a current and valid NDIS Worker Screening Check and Working with Children (WWC) Check.
